Agreements & Consents

By submitting this application, you agree to the $49 screening fee (payable directly via Rent Prep / Stessa upon receiving their email prompt). A security deposit of $500 and the first month's rent will be required prior to move-in via ACH, wire, or direct deposit.

Authorization / Consent: I authorize Ocean Avenue Health & Wellness LLC and its agents to obtain consumer reports and/or investigative consumer reports about me for the purpose of evaluating my application for tenancy. I understand these reports may include credit, criminal, eviction, employment verification and rental history. I certify that the information provided is true and complete.

Separate FCRA criminal/background authorization: I provide separate written authorization to obtain consumer reports (including criminal background checks). I understand my rights under state and federal law regarding adverse action notices.
